This rule is used to validate:
Description
PASI will not accept an update to the QA Status to “failed” if the user does not enter failure details (Failed QA Comments, Reported by User, Student Document Problem Report Type, Reported Date/Time and Reporting User Name).

Message
When this validation rule is triggered, the following message is returned:
Failed QA Information Required. When updating the QA Status to “failed” for a document, the Failed QA Comments, Reported By User, Problem Type, Reported Date/Time and Reporting User Name are required.
